There is apparently no way to make the cutout part of the component which means each cutout will have to manually be placed on the board between the LED pads. Luckily I only have 5 LEDs on the board.
However, I can't find a way to add the cutouts to the board. The closest I have been able to get is a square pad with a round hole. That won't work.
I have a 'rout' layer setup. I then place any shapes that need routing out on this layer. This is part of the component for parts but can also be used for any other cut outs required.
My pcb manufacturer knows when he sees this layer to process it in the necessary way.
Not sure if this is the correct way but it works for me.

Iain, I added a board outline in the center of the part. Is there any way to confirm the cutout is actually there? It doesn't show up in a 3D view. And when I look at the Gerber files with PentaLogix Viewmate, I don't see the cutout there either. I do see the main board outline.
I've used Iain method and the cutout can be seen on the 3D viewer. (Running V15.08). You could also add a a text Callout box on the dimensions layer clarifying the point (i.e. Qty 5 off 2.1mm x2.3mm cutouts).
I can confirm you should be able to see the cutout in the 3D viewer. When you submit gerbers to your manufacturer it would be a good idea to double check with them that the cutout info is showing.
You could try uploading your Zipped up gerbers to EuroCircuits, (http://www.eurocircuits.com) where you can use their "Visualiser" program to see a detailed view of your pcb before you place the Order with EC or your preferred supplier. Very useful!
I ended up convincing the client to change to an LED that had a round lens. That way I could add the viewing hole as an unplated drill hole and make it part of the component.
Also, I know from past experience that the board houses charge a premium for square cutouts.
