I have not produced a step and repeat board before. Is there a standard gap between the boards?

Never used this either ... normally the PCB manufacturer does it anyway..... however the normal sized router is 2mm

Depend on method of breakout. For cost effectiveness, some panels may be "V-Scored", so no gap is required. As Iain says, discuss it with your pcb supplier. If he is going to arrange the photoplotting, he can arrange the step-and-repeat too!
