Presumably the ability to create "cutouts" in Version 12 now means that no additional routing layer is needed now as in the past ?

Actually, you could do cutouts by nesting board outlines in earlier versions as well. What you couldn't do (that did require a dedicated milling layer to get round), was use slotted pads. Now in V12 you can, so special milling layers are a thing of the past.
